    • Alberto Clemente Head of the Clemente crime family in Empire Bay.
    • Brian O'Neill Irish thug who is imprisoned and becomes a target for Vito Scaletta.
    • Carlo Falcone Don Falcone, also known as "Little Humphrey" because of his resemblance to Humphrey Bogart, is a master of manipulation. Don Falcone is at the heart of the modernization of the Mafia in Empire Bay.
    • Eddie Scarpa Edorado "Eddie" Scarpa, underboss for the Falcone family. Eddie has a weakness for alcohol, women and his own jokes.
    • El Greco Not to be confused with the painter. El Greco is a doctor who provides medical services to Empire Bay's criminal underworld.
    • Francesca Vito's elder sister who lives a lawful life.
    • Giuseppe Palminteri A retired safecracker and con artist who provides Vito Scaletta with forged documents.
    • Harry Marsden An army who fought in Normandy and become a black market weapons seller in Empire Bay with an eye patch. He owns a high power machine gun and sells to Henry Tomasino.
    • Henry Tomasino A soldier and contract killer for the Clemente Family in Mafia II.
    • Jimmy A fixer and hired goon who appears in Mafia II's DLC packs.
    • Joe Barbaro Joseph Barbaro is a powerfully built, fiercely loyal man who would do anything and fight any odds to protect his friends. It's a very good thing, then, that he's the best and oldest friend of Mafia II protagonist Vito Scaletta. He also stars in the DLC "Joe's Adventures".
    • Leo Galante The consigliere to the Vinci Family in Mafia II. He's also a friend of Vito Scalletta.
    • Lisa An old friend of Vito.
    • Luca Gurino Luca is a member of the Clemente family. Luca runs a slaughterhouse on behalf of the Clemente family, where it is thought that members of rival families are taken for "questioning".
    • Maria Scaletta Maria Scaletta is mother of Vito and Francesca.
    • Marty Santorelli Marty associates with Vito and Joe. Dreamed to be in the mob!
    • Mickey Desmond Minor character in Mafia II. He is responsible for burning down Vito's house.
    • Mr. Wong Mr. Wong moved to America and became a mastermind behind the smuggling operation. Sells heroin to Henry, Joe, and Vito.
    • Sidney Pen Sidney Pen, also known under the alias of 'The Fat Man', is a minor character in the Empire Bay underworld of Mafia II. He appeared relatively frequently in pre-release trailers.
    • Steve Coyne Always reading playboy magazines. He assists Joe and Vito in taking revenge on the greasers but later he was killed together with Derek by Vito after the old dock workers reveal that Steve murdered Vito's father.
    • Thomas Angelo Thomas "Tommy" Angelo is the main protagonist of the original Mafia game. He is a taxi driver who eventually becomes a made man in the Salieri crime family.
    • Vito Scaletta Vito Scaletta is a central character in the Mafia series. He is an Italian-American war veteran who is tied with several Mafia families.
